The spectrum of microbial pathogens in cystic fibrosis (CF) lung infections differs considerably from that of other patients with chronic lung disease. Due to the chronic course of lung disease, bacterial pathogens change their phenotype and mucoid or small colony variants are often observed. These pathogens are not easily recognized by laboratories not specialized in CF microbiology. In addition, the stress of the local lung environment leads to the occurrence of hypermutable bacterial strains, which show a large variety of genotypic and phenotypic traits including resistance to antimicrobial drugs. What should be done to detect and not overlook any of important CF pathogens, which methods can be used in addition to culture to timely detect and identify them, which isolate characteristics are important to report to clinician and how to perform antimicrobial susceptibility testing for CF pathogens are among the most burning questions to be answered.
